Counter-Notice
If you believe that the material was removed by mistake or misidentification, you may send us a counter-notice containing the following information:
Your name, address, and telephone number.
The URLs or other identifying information of the material that has been removed.
A statement under penalty of perjury that you have a good faith belief that the material was removed as a result of mistake or misidentification.
A statement that you consent to the jurisdiction of the federal court in your district (or if you’re outside the U.S., in the jurisdiction of [Company’s Location]) and that you will accept service of process from the person who submitted the original DMCA notice.
